How to Apply

The 2017-2018 application cycle will open October 15, 2017. Please check back September 2017 for application updates and changes.

IMPORTANT DATES: THE ENTERING CLASS OF SUMMER 2017.

Applications Open: October 15, 2016

Recommended Deadline: February 15, 2017

Application Final Deadline: May 1, 2017

*Application and all supporting documents must received by this date.

Program begins in the summer on mid JUNE 2017.


 

Application and supporting documents mailed in one packet:

  1.  DOWNLOAD APPLICATION
    • Complete and print application
    • Essay Prompt
    • Application Acknowledgements sign (last page of application)
  2. Progress Expectancy Contract  – print and sign
  3. Transcripts
    • Official transcript(s) in their original sealed envelope from all college institutions attended

Three letters of recommendation from

  • Request letters of recommendation from the following letter writers:
    • A college instructor
    • Community or clinical service experience, or employer
    • Letter writer of your choice ( letters from family and friends are not accepted)

 

  • Provide all letter writers with a completed  Statement of Confidentiality Waiver Form.
  • At least one of the letter of recommendation is directed towards admissions into our program.
  • Letters should be typed on official letterhead, signed, and dated.
  • Letters must be original. We can not accept copies or scanned images.
  • Letter writers will need to mail the letter with the Statement of Confidentiality Waiver Form to our admission address (shown above).

Application Processing Fee (non-refundable) – $100

Complete the Application Processing Fee Payment Form. Payment form can be submitted by mail or fax.

Utilize the Application Check List to help ensure you have completed all the required steps to successfully submit your application.
